Many, but apparently not all, of the Biosentinels were members of Simon Trask's "Humanity Now!" activist coalistion. Trask also infected other humans who were not part of that coalition. Once infected with a nano-virus Trask spread, humans "mutated" into mechanical humanoids completely under Trask's control. All vestiges of the original human's thoughts and soul were gone, replaced by the Biosentinel and it's programming.

After Emma Frost telepathically scanned the Biosentinels, she told her teammates on the Dark X-Men team: "Whatever Trask infected them wth has killed them. These things are no longer human. They're no longer technically alive. Stop them however you deem appropriate."
