Page 2 of X-Factor #50 (Dec. 2009) briefly describes the "origin" of Anthony Falcone, the Presidential advisor who (80 years in the future) will try to destroy all mutants from the face of the Earth. The narrative captions (apparently narrated by an older Layla Miller to her younger self) describe Falcone's history. This brief history mentions the place of his parents and grandmother in his story and in the formation of his deep hatred of mutants. The narrative captions on page 1 read:

"Once upon a time -- forty years from now -- there was a boy who lived in an orphanage, just as you do now. His parents were killed, and his home destroyed (he was off visiting his grandma that night) under 'mysterious circumstances.' That's what authorities always called it when they thought mutants were involved. The boy's name was Anthony Falcone, and he hated mutants. Hated them more than anybody ever had, maybe."

Technically speaking, we can't be sure that this woman's last name is "Falcone." She is described as Anthony Falcone's grandma, but if she his maternal grandmother, then her name would not be "Falcone."
