Cain Marko (Juggernaut) was in a bar where his step-brother Charles Xavier showed up and talked to him. They were there along with many patrons and some workers (bartender, etc.) Paul Meacham was the only other person there identified by name. The Juggernaut forced Xavier to sit and have a drink with him by physically threatening Paul Meacham, who was just an innocent bystander in the bar.

It turned out that this scene was all in Cain Marko's mind.

It is likely that Paul Meacham was a real person who Cain Marko (the Juggernaut) remembered. But when Meacham was depicted in X-Men: Legacy #219, he appeared only in a bar in Marko's mind. This memory (or illusion?) was all part of an experience that Charles Xavier mentally subjected the Juggernaut to in order to discern whether or not he was returning to his past evil ways.
