The first published appearances of "Johnny C." (perhaps better known as "Johnny the Homicidal Maniac") were in a regular comic strip titled "Johnny the Little Homicidal Maniac," written and drawn by Jhonen Vasquez. The comic strip appeared in the student newspaper of Vasquez's school, Mount Pleasant High School.

At this point, we have not yet found the precise publication date of the first comic strip featuring this character. But given the fact that Vasquez was born on 1 September 1974 and published the strip while he was in high school, but original publication date would be in or very close to 1990.
