This comic was reprinted from: Captain America (vol. 1) #144 (Dec. 1971): "The Falcon Fights Alone!".

This publication is also known as: Captain America and Falcon #1. Most sources identify the title of this publication as Captain America and Falcon #1. This is probably the title as given in the indicia. The title on the logo is clearly Captain America and the Falcon #1.

Notes from original source:

Note that "The Falcon Fights Alone!" is identified as "Chapter Two" in Captain America (vol. 1) #144. This story has a distinct plot from "Chapter One" ("HYDRA Over All!") and a distinct artistic team, but it continues the page numbering from Chapter One. Thus, "The Falcon Fights Alone!" begins on page 11.

John Romita penciled and inked the very last panel of this 10-page story. Other than that, Gray Morrow did all of the penciling and inking of this story's artwork.
